Acute Link Nurse *with £3000 welcome bonus*
West Norfolk Acute Services are looking to recruit an experienced Band 6 Link Nurse to work between our acute mental health teams in KingsLynn. This job role will focus on improving patient flow through admission prevention and facilitating early discharge from the inpatient service through care planning and delivering intensive home treatment options.
We offer an acute crisis pathway to improve the patient journey and overall experience of service users, families and carers. The post holder will liaise with internal and external teams to ensure a smooth transition between teams and family/carer liaison where appropriate.

Main duties of the job
This job role involves working Monday to Friday 09:00am to 17:00pm but every day will be different. You may attend inpatient reviews, undertake interventions on a 1:1 basis such as Dialog + (full training provided), support the CRHT in assessment and treatment planning or attend MDT and professionals' meetings. You will build on existing internal and external relationships to ensure that our service users are supported by the right people to optimise their recovery. You will work proactively with service users to ensure purposeful admissions to the inpatient unit and identify goals and barriers to discharge early on. You would work closely with our Discharge Facilitator.

Person Specification
Qualifications
* Nursing degree or equivalent, Dip OT, Clinical Psychology or equivalent
Experience
* Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
* Demonstrate effective risk assessment and risk management experience
* Experience in working in team delivering person centred treatment packages
* Experience in delivering group based interventions
Skills
* Effective analytical and decision-making skills
* Risk assessment knowledge and skills
* Care planning skills
Knowledge
* Knowledge of medication and the monitoring of its effects
