Job Responsibilities To undertake medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly To support the achievement of the Practices prescribing objectives and targets including Quality and Outcomes Frameworks, Local Quality Contract, Cost Saving and Enhanced Service Specifications as directed To undertake telephone consultations with patients, in the context of medication reviews and other clinical services relating to Medicines Management Working with the Clinical Pharmacist to contribute to the review and development of repeat prescribing protocols / standard operating procedures to improve the efficiency and effectiveness of repeat prescribing systems To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness To participate in medication audits To assist in the appropriate monitoring and management of prescribing budgets and to liaise with the medicines management team at the ICB as needed and required To support the Clinical Pharmacist to provide medication reviews, patient information leaflets, medicines awareness and to undertake engagement with patients and local PPG groups as needed To undertake and oversee programmes for patients in managing their medications more effectively, considering whether repeat dispensing / meds synchronisation would be more appropriate and applying where able. To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice guidelines and protocols To assist in the submission of claims for personally administered drugs as required. To support the Senior Clinical Teams with the review of hospital-initiated medication changes in accordance with practice protocols Highlight any medication errors / queries, reporting any concerns to the Clinical Pharmacist / GP Partners for review Ensure that the medicines management recalls are carried out / oversee that the Prescription Clerk teams are up to date with required monitoring in line with prescribing and any other annual recalls as are due. To oversee and support the medicines management workflow systems acting upon requests and ensuring that the workflow of the systems is working effectively, and correspondence is being actioned timely. Provide training for practice staff on systems for medicines management / prescribing systems and help develop any required prescribing protocols. Provide support the New Patient Registration process in ensuring medical records are obtained from the previous surgery timely, any recalls for medicines management are initiated and their medicines management aspects are transferred as timely as possible Support the Training Practice elements of the Practice by way if assisting in the induction programmes of medical students / placements / trainees, providing an overview of the practice prescribing policies and guidance as is available
