PA required to support a mature woman from Llanelli who is severely visually impaired to remain in her own home with dignity for 6.75 hours per week. She requires this support in order to minimise the risks involved. She would be at high risk of falls within the home and a risk of a further depression. Autonomy and choice are important to her, her views should be considered at all times wherever possible throughout the care process.
She has an intercom system in place for the front and back door. There is a lifeline in situ to alert family or any emergencies at home. She requires a trolley in the home and a wheelchair for outside of the home, for safe mobilising. She can be unsteady on her feet, she walks very slowly. There is a wet room in situ with a shower chair. she has a perching stool to undertake personal care with support. There is also a stair lift in situ.
Principal duties and responsibilities:
1. Ensure that personal care and hygiene is maintained at all times.
2. Skin integrity to be preserved wherever possible.
3. Ensure environment is free from hazards and safe.
4. Supporting the employer with encouragement in gaining independence
5. Enabling the employer to interact socially in their community
6. Optimum fluid and nutrition intake will be maximised.
7. Maintaining family contacts.
8. Minimise anxiety.
9. Prevent carer's role from breaking down
10. To provide companionship in the employer’s home, including engaging in conversation with the employer and providing them with social interaction
11. Encouraging the employer to attend appointments with support
12. Ensuring her safety at all times during your working hours
13. To carry out any other reasonable tasks that are requested by the employer during your working hours
14. To ensure compliance with Data Protection Act.

16. Be responsible for contacting the GP, Social Worker or Local Authority should the employer need medical assistance.
17. To liaise and discuss with her family any concerns and report on activities undertaken

9. A full, clean driving licence and use of a car is likely to be required. This is determined by the needs of the employer and whether you live within walking distance.

11. Be prepared to undergo training in order to best meet the employers need.
