An exciting opportunity has arisen for an experienced Nurse to work with the existing Nurse Practitioner, Renal F Floor Team, and Transplant/Living Donor Coordination and Surgical Team.
The jobholder will work with the existing Renal Transplant Nurse Practitioner and Renal F Floor Team responsible for the long-term clinical care of Sheffield Teaching Hospitals NHS Foundation Trust renal transplant patients.
The jobholder will have responsibility for educating and reviewing all post-transplant patients on the Renal Transplant ward/ward rounds (RUF) post-transplant and will review in clinic up to the patient joining the renal transplant main clinic appointment system. These clinical visits are intended to compliment normal outpatient visits with medical staff.
The aim will be to spend more time recapping post-transplant educational needs such as medication review and supporting the patient with psych-social needs including sign posting to relevant specialist services such as renal psychology, renal social work, and patient charities such as Sheffield Kidney Patients Association (SAKA).
To provide support to the team including performance development reviews, clinical supervision, audit, education, and training related to renal transplant patient care/ management.
Applicants should be enthusiastic and well-motivated with relevant extensive experience in Renal Care.
Main duties of the job
The jobholder will work with the Renal F Floor Team responsible for the long-term clinical care of Sheffield Teaching Hospitals NHS Foundation Trust renal transplant patients.
The jobholder will have responsibility for educating and reviewing all post-transplant patients on the Renal Transplant ward/ward rounds (RUF) post-transplant and will review in clinic up to the patient joining the renal transplant main clinic appointment system. To work with medical staff in the capacity of nurse support to all new recipients of renal transplants.
To undertake the comprehensive assessment of patients, post renal transplant, and to formulate individualised education management and treatment plans.
To participate in multidisciplinary working.
To work across professional boundaries, proving comprehensive health care that is sensitive to the needs of patients.
To continually monitor, assess and evaluate treatment outcomes, and adjust treatment plans, accordingly, focusing on a self-management approach whilst appropriately referring to other agencies as necessary to meet patients’ needs.
To receive and communicate complex, sensitive, or contentious information to patients and carers to maximize the self-management potential and to ensure understanding of condition.
To establish links to a range of communication skills to ensure effective communication with all patients including those who may be hard of hearing, of ethnic origin or with learning difficulties. To communicate effectively with patients and their carers when they require empathy and understanding for e.g., if patient’s kidney transplant fails straight away and they require dialysis support thereafter.
To communicate effectively and work collaboratively with health and social care colleagues as necessary, to ensure renal transplant patients care and management is delivered as part of a co-ordinated multi-disciplinary service to patients within the complexities of a community environment.
To establish and maintain robust communication networks with patients, carers and other health workers and agencies.
To train, supervise and performance-manage junior staff and students in a clinical, professional, and organisational capacity.
In line with local guidelines, review and reflect on own practice and performance through effective use of professional and operational supervision and appraisal.
To demonstrate a high level of ongoing personal development through participation in internal and external development opportunities, recording learning outcomes in a portfolio.
In accordance with Post-Registration Education and Practice (PREP), support professional profiling and reflective practice, ensuring own and colleagues’ educational requirements are met.
Take personal responsibility for own development including mandatory updates, appraisal, accounting for PREP and Scope of Professional Practice.
Respond promptly to emerging clinical needs, complaints and incidents and develop and participate in appropriate action plans, in line with Clinical Governance.

1. RGN /RN on part 1 of the Nursing and Midwifery Council (NMC) Professional Register.
2. To have completed the national PEAP programme for practice supervisors or equivalent teaching and assessing qualification for those with ENB998.

3. First degree.
4. Counselling skills course.
Experience
Essential criteria
5. Extensive relevant experience in Renal Care.
6. Current/Previous experience caring for patients post renal transplant.
7. Ability to teach patients and staff.
8. A working knowledge of the needs of patients following renal transplantation.
9. Computer literate with excellent written and oral communication skills.

10. Experience of clinical audit and data collection.
