Job Description Summary:
Join the OhioHealth team as an Occupational Therapist! Experience one on one patient care, a flexible schedule, and a great benefits package. Home health OTs work with an interdisciplinary team to care for patients in the home setting. Join our amazing home health team where you have autonomy, a reasonable and fair weekend rotation, a flexible schedule, and limited on-call. Home health OTs provide skilled care to patients to achieve outcomes in collaboration with the patient’s provider.
Responsibilities And Duties:
1. Driving is an essential function of the job.
90% Direct Patient Care - Evaluates and treats patients after physician referral to:
1. Evaluate patient’s motor, cognitive, sensory integrative and/or psychosocial function.
2. Develop and implement plan of care based on evaluation findings and appropriate treatment techniques.
3. Implement plan of care based on appropriate occupational performance frame of reference(s): bio-mechanical, sensorimotor, and/or rehabilitative.
4. Attend multidisciplinary patient care meetings. Plan and coordinate discharge plan with multidisciplinary team.
5. Educate patient and caregiver/family.
6. Recommend Medical Equipment required for discharge.
7. Provide information to physicians, nurses, case managers, social workers, and other team members.
5% - Document evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties.
5% - Indirect Department Functions:
1. Supervise patient assignments and duties of Certified Occupational Therapy Assistants (COTA) and rehab technicians.
2. Be involved in department quality improvement through utilization review and quality assurance program.
3. Take active role in orienting new staff members.
4. Serve as clinical instructor for OT and COTA students.
5. Educate other disciplines and medical/hospital personnel on the topic of OT purpose, goals and treatment policies and procedures.
6. Represent department at problem-solving and committee meetings.
7. Assume other duties delegated by management.

Minimum Qualifications:
* Bachelor's Degree (Required)
* BLS - Basic Life Support - American Heart Association
* DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles
* LIC - Licensed in Field - State of Ohio
* OT - Occupational Therapist - American Occupational Therapy Assoc.
Additional Job Description:
