Job summary The Focus Care Practitioner aims to deliver support and to utilise psychological skills within the MDT. The practitioner will work in collaboration with the patient to deliver care plans aligned to recover focused outcomes. The practitioner will have direct patient contact in the form of assessment and treatment. Our practitioners within the Focused Care Pathway will be working with specific patient groups, who are recovering through different pathways. Some will require time-limited brief psychologically informed interventions leading to discharge. The post holder should have passion for developing their clinical skills and be keen to work flexibly with a diverse caseload in a variety of settings. Main duties of the job To participate in weekly team meetings, communicating service user needs and be able to assist in prioritising the allocation of workload. To contribute to the weekly multi-disciplinary meeting where service user's presenting with an increased level of need are discussed and to present cases, on a rota basis, for team review. To formulate care plans in line with identified need and review in line with the Trust Care Programme Approach and Care Planning Policy. To provide clinical supervision and caseload management to support workers. To provide therapeutic interventions in line with the agreed care plan and risk assessment, utilising psycho-social interventions and CBT based treatment To assess risk, develop risk management plans, review and adjust in line with the service user's needs. To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user. To use Dialogue Plus Care Plans for structured work and to focus on recovery.
