Job Summary
This is a new post to support the delivery of NHS England Pan-London Sickle Cell Improvement Programme, through West London ICB with investment into improving haemoglobinopathy services in primary and secondary care at London North West University Healthcare NHS Trust and Imperial College Healthcare NHS Trust. The post is a fixed term contract for 24 months.
Provide specialist nursing and support services for adults with sickle cell, thalassaemia and other haemoglobinopathies in community care settings. Assist the Nurse Consultant for Haemoglobinopathies and Community Clinical Nurse Specialist Team Leader - Haemoglobinopathies in reviewing, formulating and implementing strategies to meet service demands, manage appropriate levels of nursing and other staff, if appropriate.
Maintain professional links with health and allied professionals working in the acute and community sector and with statutory and voluntary agencies. Work within the remit of the NMC code of professional conduct.
Key Responsibilities
1. Provision of specialist, high quality, expert nursing care and support for patients and their families / carers with haemoglobinopathies trait/disease.
2. Deliver evidence based individualised patient care according to patient’s changing health care needs.
3. Work autonomously and seek collaborative relationships with others across and within professional boundaries.
4. Provide expert knowledge and advice in own specialist area, ensuring maintenance of clinical excellence underpinned by evidence-based practice.
5. Work with the multidisciplinary team and management team to ensure that the service complies with the national standards for safety, quality and ensure the service complies with the key performance indicators aligned with the NHS England Sickle Cell Improvement Programme.
Main Duties and Responsibilities
Clinical Responsibilities
1. Act as a clinical expert within your sphere of work.
2. Demonstrate an understanding of haemoglobinopathies, the implications of living with a chronic illness and its impact on individuals, families and wider community.
3. Develop clinical nursing competencies within your expert sphere encouraging the development of new and novel roles within your service.
4. Demonstrate expert knowledge being visible, accessible and using advanced clinical skills within the workplace as appropriate.
5. Carry a clinical case load across primary care settings, provide support for secondary care colleagues.
6. Establish a partnership approach to care, empowering and educating adults, their carers and their families to actively participate in delivery of care.
7. Provide expert advice on issues related to the specialty internally and externally.
8. Use knowledge and skills to advise adults, carers and their families on symptom management and care.
9. Support the transition of adolescents to the adult services within the Trust.
10. Liaise with district/community nurses, and other members of primary healthcare team to ensure ongoing support of the patient, carers and family.
11. Supervise and ensure a consistent approach to nursing across all areas covered, through collaborative working and the development of shared procedures and guidelines.
12. Be responsible for the assessment, escalation and management of risk in accordance with the Trust’s Risk Management policy.
13. Keep informed of national and Trust policies, especially those affecting service delivery and patient care.
14. Provide clinical cover when required within the specialist services.
Managerial / Leadership Responsibilities
1. Provide support for Community Clinical Nurse Specialist Team Leader Haemoglobinopathies as required.
2. Participate in preparation and delivery of key performance indicators linked to the NHS England Pan-London Sickle Cell Improvement Programme.
3. Promote a culture that supports patient centred care and patient empowerment.
4. Act as a role model demonstrating high standards of care and providing clinical leadership to others.
5. Participate in the recruitment and induction of new staff.
6. Collate and interpret quantitative and qualitative data to provide evidence of productivity, outcomes and quality.
7. Ensure that all adult haemoglobinopathy activity is recorded on Trust electronic record system.
8. Contribute to the identification of service goals and appreciate how these link with key performance targets and quality indicators.
9. Actively participate in multi-professional meetings, acting as the patient advocate and representing nursing and patient views.
10. Work closely with the other Clinical Nurse Specialists within the Haemoglobinopathy Nursing Team and provide cross cover if/when necessary.
11. Ensure work links to the goals and objectives of the haemoglobinopathy service.
12. Attend and participate in organising local, national and international networks and forums relating to sickle cell, thalassaemia and related conditions.
Education

3. Provide specialist education and training to other professionals involved in patient care.

6. Act as a mentor for newly appointed Clinical Nurse Specialists.
7. Access clinical and safeguarding supervision.

1. Influence and participate in development of policy and clinical guidelines.
2. Work closely with colleagues to develop evidence-based practice within this area of nursing.
3. Lead the development and facilitate regular reviews of all protocols and guidelines specific to haemoglobinopathies.
4. Promote the service and clinical excellence by publishing and presenting innovations, audit and research locally and nationally.
5. Participate in relevant local and national audit for specialist group.
Person Specification
Education/Qualifications
Essential Criteria
1. 1st Level Nursing Registration
2. Current registration with NMC
3. 1st Level Degree in Nursing or evidence of working towards
4. Post registration qualification appropriate to or transferable to haematology/haemoglobinopathies
5. Evidence of further professional development or working towards
Desirable Criteria
1. Working towards Masters level qualification
2. Community nursing qualification or working towards
3. Leadership or mentorship course
4. Recognised teaching qualification or working towards
Knowledge and Experience
Essential Criteria
1. Experience in sickle cell and thalassaemia nursing – acute or community
2. Clear understanding of safeguarding policies.
3. Evidence of applying current research into practice
4. Experience of working with minority ethnic groups in multi-cultural setting
5. Previous experience of teaching and developing teaching materials
Desirable Criteria
1. Community nursing/District nursing experience
2. Experience in genetic counselling
3. Previous experience of working with a specialist haemoglobinopathy service
4. Evidence of advanced communications skills
