Summary
This Physician Assistant position is in the Primary Care Service Line at the Jamaica Plain VAMC in Boston, MA. This position is full-time at 40 hours per week.
Duties
* The Physician Assistant (PA) is accountable for providing Primary Care and follow-up care for a prescribed number of patients with an emphasis on maintenance and promotion of optimal wellness and the coordination of care across the continuum of health care.
* The PA is accountable for adhering to the established policies, standards of care and practice, and achieving the goals for the Clinical Guidelines and Prevention Indicators.
* He/she utilizes advanced skills in assessment, problem identification and clarification, treatment planning, and implementation of the patient's treatment plan.
* The PA will have prescription authority with a scope of practice.
* He/she provides care for men and women veterans ranging in age from 18 through the geriatric years, taking into consideration diagnosis, psychosocial, spiritual, and cultural issues.
* The PA works in collaboration with other members of the health care team to design and/or continuously improve the quality of care delivered to veterans.
* Duties include but are not limited to: Provides coordinated comprehensive primary care to a prescribed number of patients, including referrals to specialists as indicated.
* Facilitates the ordering of diagnostic tests, i.e., lab work, x-rays, EKGs, consults, and similar studies.
* Documents in the medical record all procedures performed for which the PA is responsible.
* Documents clinical findings and progress in the medical record consistent with timeliness and appropriateness standards of practice and established policies.
* Formulates a comprehensive database which includes a physical and psychosocial health history, physical examination, and evaluation of laboratory diagnostic testing, as indicated.
* Initiates and monitors health maintenance in conjunction with the patient, family/significant other, and members of the multi-disciplinary team.
* Discriminates between normal and abnormal findings in planning treatment intervention therapies.
* Collaborates with the supervisory/attending physician regarding abnormal findings.
* Initiates and adjusts selected medications with formulary approval if indicated for patients.
* Initiates and adjusts the plan of care and emergency treatment when indicated.
* American Heart Association guidelines (CPR) will be followed.
* Stresses education and rehabilitative goals, and health education, emphasizing patient and family/significant other participation in planning, treatment, and goal formulation.
* Provides and prompts psychological support, in collaboration with interdisciplinary team members, to patients and their support network.
* Participates in interdisciplinary team conferences to assure the highest quality patient care and panel management.
* Acts as a consultant to nursing personnel, physicians, and other health care consultants in their area of clinical experience.
* Initiates and participates in performance improvement that results in improved outcomes.
* Identifies, initiates, or participates in selected research studies related to patient care.
* Maintains knowledge of current techniques, trends, professional issues, as well as information technologies.
* Ability to communicate and work effectively with other members of the team.
* Contributes to the formation of health policies or programs that make a sustained contribution for the health care of patients/significant others.
* Provides clinical back-up and cross coverage of other providers' patients.
