Job Summary:
Intermediate Care @ Home aims to support people with health and social care needs to stay in their own home rather than being admitted to hospital or into long term care. It provides support to people when they are discharged from hospital, helps people to remain independent and helps to identify any ongoing needs.

Main Duties of the Job
Hours: 37.5 - (Full-Time / Part-Time roles available), flexible rota pattern between 7.00 am and 10.00 pm, including weekends and Bank Holidays (Permanent)
Salary Range 4 (£24,645 - £27,202), fixed salary point
We are seeking to recruit enthusiastic and committed member of staff to work within a CQC Outstanding established and integrated enablement service with an excellent reputation. Providing short term high quality services to people within their own homes across N E Lincolnshire the service is dependent on having a robust multi-skilled team responsible for ensuring we get the best possible outcomes by working in partnership with service users; carers; families and our health colleagues. The role also includes working with therapy programmes, ensuring that we promote an individual's independence at all times.
The Assistant Practitioner is responsible for providing a varied range of health and social care services to service users out in the community which will include undertaking regular clinical observations of service users and recording accurately; reporting any unexpected changes in condition to the appropriate professional and/or GP; administering and accurately recording medication; carrying out basic level clinical interventions e.g. catheters, pressure area care, continence care and assisting service users with daily living activities. You will also be responsible for the day to day supervision of the staff team.

Minimum Requirements
Experience Essential
* Experience and competence in use of Microsoft Office applications: Word, Excel, Outlook and Access
* Previous experience of working in a health and/or social care environment
Desirable
* Experience of working within a rehab and re-ablement environment with adults 18 years+
Other Essential
* Ability to demonstrate commitment to CPG core values
* Ability to transport self, others and equipment around North East Lincolnshire to prescribed deadlines
* Able to work evenings, weekends and bank holidays
Qualifications Essential
* NVQ/QCF Level 4 in Health and Social Care
Desirable
* Care Certificate
* Medication Certificate
