This is a Part Time role for hours per week,working hours Thursday, Friday and Monday.
Anexciting opportunity has arisen for a part time Patient Pathway Co-Ordinatorfor the Directorate of Vascular Surgery.
Thiswill involve co-ordinating and providing administrative support to the Vascularand Cardio-Vascular multidisciplinary team, co-ordinating seminar clinics,collate and record accurate and timely data, provide up to date information onpatients pathways ensuring any barriers or delays in treatment areremoved. To monitor all waiting listsfor the directorate and to support with the booking of theatre lists. You will work closely with ConsultantSurgeons, Specialist Nurses, Deputy Admin Manager and Personal MedicalSecretaries.
Applicantsmust have excellent organisational skills, be extremely efficient and be ableto work under pressure as part of a large team and to tight must be able to prioritise and manage a busy workloadand be committed to delivering the best care to (Grade4 9 or equivalent in Maths and English and NVQ Level 3 (business admin/orrelevant experience) are needed. Knowledge of MicrosoftOffice Packages (including Outlook and Excel) is essential and knowledgeof medical terminology/medical administration is desirable. You should have anunderstanding of the 18 week RTT standard and application of rules throughstrict guidance.
Main duties of the job
The main dutiesof the role will be to compile a list of patients for discussion during theVascular MDT and attend the meetings.
Coordinatemeetings, dates, rooms and agendas and document MDT discussions in Medtechrecording these live.
Provide updateson patients pathway and ensure all MDT outcomes are acted upon by working withthe vascular secretarial team.
Communicatestandard letters to patients.
Validation orwaiting lists and reports and booking of theatre lists.

Overall Job Purpose:
To monitor all waiting lists and patientpathways for the Directorate, to ensure all patients are managed within localand national referral to treatment timescales, validating pathways in realtime.
To monitor all patients referred to theDirectorate under a Cancer pathway referral, to highlight any areas of concernin the pathway and to ensure the cancer waiting times are achieved. This willinvolve working closely with Cancer Services Team
To provide an early warning system toDirectorate Managers regarding waiting times, demand and capacity issues withinthe Directorate.
To use the Trusts Launchpad, to review and monitorpatient pathways, working with information services and the performance team,to ensure robust processes are in place to monitor and highlight performancefor the directorate.
To work without direct supervision andexercise significant initiative and independent judgement in managing thislarge area and workload.
To liaise with consultants, personal medicalsecretaries, directorate managers and the Cancer Services team, to ensure allpatients are managed within 18 weeks/national waiting times and where this isdeficient, participate in the development of actions to rectify the position.
