To ensure that data is validated at all points on the patients pathway. To review patient records and use information within to validate RTT /Non-RTT data extracted from the PAS system and provided by the Information Team. To input data collected on outcome forms onto the PAS system within 48 hours. Ensure that patient admissions and discharges are recorded accurately and in a timely manner on the PAS system. To promptly report any identified data quality issues or potential wait time breaches to the Validation Team manager/ Team Lead/ Data quality Lead.
To liaise with relevant staff to obtain information needed to appropriately validate pathways in accordance with 18 week/26 Weeks RTT Clock Rules. To action appropriately in order to prevent patients from breaching the 18/26-week target. To support staff in their understanding and application of the 18-week/26 Week pathway and to act as a point of contact on any RTT related queries. To work closely with the units, checking patient pathway details have been recorded accurately on our systems.
To maintain an accurate record of the number of pathways validated and ensure comments are entered next to each record to efficiently act as an audit trail. To support the Validation Team Leader/Manager in ensuring that all diagnostic and therapeutic elements of the pathway are captured and recorded. Also making sure that lead time is correct for patients on a Non-RTT pathway and no duplication are made. To use and update multiple clinical information systems and databases to understand and track the patient pathways. To unravel patient pathways in an attempt to understand the various stages of the patients care and pathway.
To source, collect and validate accurate RTT/Non-RTT data in accordance with agreed practices and standards. Support the Admissions staff and Ward Clerks in ensuring accurate and timely recording of admissions and discharges within 48 hours of the event. Code of Conduct - All staff are expected to adhere to policies and procedures in the Trust which establish standards of good practice and follow any codes of conduct which are relevant to their own profession. To participate in planned training events and to attend all mandatory training sessions as requested. To be able to apply the 18-week/ 26 Week rules to different patient pathways across a wide spectrum of specialities.
Ensure the accurate and efficient validation of open pathways and Non-RTT pathways. This involves close working with clinical staff, local management teams, other validation staff, line manager and corporate information team. To record accurately the reasons for patient breaching the 18/26 weeks standard and monitoring the overdue long term follow up patients. Make decisions demonstrating specialist knowledge to support the production of high quality data on the Trusts PAS.
Confirm the accuracy of the RTT data. Report accuracy issues back to the user in a structured and informative way, enabling the users to improve their knowledge of the patient pathways and data entry on PAS. To collect additional data on an adhoc basis as required. To plan and organise own work schedule with the use of appropriate planning aids, demonstrating good time management and ensuring anticipated difficulties in meeting deadlines are promptly reported.
Guide services in regular routine validation of waiting lists to ensure the waiting lists is accurate at all times. Attend meetings as required within role to support the Validation Team Leader/Manager. To keep skills up to date and relevant in order to carry out appropriate tasks in relation to the core information systems and desktop applications. To promote the department in a positive manner at all times.
