Main duties of the job
You will be expected to integrate into the multi-disciplinary team and work closely with the Clinical Pharmacist to maximise medicines optimisation duties, which will include drug history taking, patient counselling, co-ordinating the supply of discharge medication from wards, audit, and training of multi-disciplinary team members in medicines management issues.

The post holder will:
1. Provide the medicines management service to designated ward areas as part of the clinical pharmacy team, which involves following relevant medicines management procedures to deliver the service in a safe and effective manner but working unsupervised in clinical areas.
2. Perform drug histories on admission areas and in other areas on request.
3. Assess patients' own drugs to ensure they are labelled appropriately and meet required standards for administration.
4. Provide appropriate supplies of inpatient medicines for bedside administration (stock and non-stock medicines).
5. Initiate supply of non-stock items for one-stop dispensing, i.e., ordering medication labelled ready for discharge when appropriate and ensure that sufficient supplies of medication are available in each patient's bedside locker.
6. Co-ordinate discharge prescriptions bringing together patients' own drugs and newly dispensed items in advance of patient discharge.
7. Counsel patients regarding their medicines to ensure patients understand their medication regimes including benefits of their medication and possible side effects.
8. Communicate discharge information to patients, carers, and GPs as necessary in line with departmental policy.

* NVQ level 3 in Pharmacy Services / BTEC in Pharmaceutical Sciences or equivalent
* Registered with the General Pharmaceutical Council of Great Britain
* Accuracy checking accredited certificate
* Experience of working as a qualified technician
* Working as an accuracy checker in a dispensary or on a ward/other clinical area
* Pharmacy labelling and stock control systems

* NVQ A1 workplace assessor
