Key Responsibilities
* To lead and manage a team of care staff, providing guidance and support to ensure the delivery of exceptional care and services.
* To assess and develop individualized care plans that cater to the unique needs and preferences of our service users.
* To monitor and evaluate the effectiveness of care programs, making recommendations for improvement and implementing changes as necessary.
* To maintain accurate records and reports, ensuring compliance with regulatory requirements and organizational standards.
* To collaborate with other departments and external agencies to secure resources and services that support the well-being of our service users.
Requirements
To be successful in this role, you will need:
* A degree in Social Work or a related field, or equivalent experience in a care management capacity.
* Proven experience in managing care teams and developing care plans that prioritize the needs and preferences of individuals with learning disabilities.
* Excellent communication and interpersonal skills, with the ability to build strong relationships with colleagues, service users, and families.
* Knowledge of relevant legislation, policies, and procedures governing the care of individuals with learning disabilities.
* A commitment to ongoing professional development and a willingness to adapt to changing circumstances and priorities.
