Proficient conversational skills in spoken English as per Fluency Duty requirements. What the Job Involves Managing a varied and complex caseload while maintaining professional accountability. Conducting needs assessments following the Care Act 2014 and promoting a strengths-based approach. Involvement in safeguarding enquiries and placing outcomes at the center of practice. Acting as a deputy in the absence of the Lead Practitioner. Supervising and mentoring up to four non-registered social care practitioners and social work students. Identifying and addressing non-social care needs and collaborating with partners. Constructing value-for-money care packages in line with eligibility criteria. Assisting individuals, families, and communities in informed decision-making and service planning. Researching and reporting on service provision and resource gaps. Maintaining case notes, writing reports, and possibly giving evidence in court for care proceedings. Ensuring compliance with Lincolnshire County Council Adult Care policies and procedures. Promoting organizational transformation and self-service for cost effectiveness. Safeguarding children and vulnerable adults, respecting relevant policies and procedures.
