An exciting opportunity has arisen for a Clinical Triage Co-ordinator to join the South West Interdisciplinary Team in the provision of Immediate and Planned Care to patients in the community. The multidisciplinary teams provide nursing and therapy services offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential and functional independence. Your role will include dealing with referrals to the nursing and therapy team, general administration work & supporting the Team Leaders. Main duties of the job To deliver call handling and support to the triage function of the community nursing and therapy team. To support the administration functions of the community nursing and therapy team.
