Main duties of the job
As a Nurse in the frailty team, you will provide high-quality nursing care to frail and elderly patients, focusing on managing long-term conditions, wound care, and supporting proactive hospital discharge reviews. You will work within a multidisciplinary team, liaising with GPs, nurses, paramedics, pharmacy technicians, and voluntary services to ensure integrated care for complex patients.
Job responsibilities
1. Deliver high-quality nursing care to frail and elderly patients, focusing on long-term condition reviews, wound care, and frailty management.
2. Work with colleagues in the frailty team to identify patients who would benefit from proactive review, using the What Matters to Me model.
3. Provide wound care support, especially when district nursing services are not accessible.
4. Conduct proactive reviews for patients discharged from the hospital, ensuring safe and effective follow-up care.
5. Assist with annual vaccine campaigns for frail patients, including flu & covid vaccinations.
6. Build strong working relationships with GPs, paramedics, and other members of the frailty team to ensure integrated care.
7. Liaise with community partners, such as district nursing, palliative care teams, the voluntary sector, and the Integrated Care Therapy team, to improve patient and carer outcomes.

* Meet the requirements and produce evidence for nurse revalidation

* Car driver with access to a vehicle to complete home visits
Qualifications
* Registered Nurse (Nursing and Midwifery Council)
* Mentor or teaching qualification
* Postgraduate diploma or degree (Chronic Disease Management, case management, primary care)
* Nurse prescriber
* Frailty/older people/dementia/palliative care-specific training
Experience
* Experience of working in a primary care environment
* Experience of chronic disease management
* Experience of working autonomously
* Experience of working as a practice nurse or community nurse
* Experience of managing frail/elderly and housebound patients
* Experience of quality initiatives, i.e., benchmarking
Clinical knowledge and skills
* Wound care/removal of sutures and staples
* Venepuncture
* Medication Reviews
* Requesting pathology tests and processing the results, advising patients accordingly
* Understanding the importance of evidence-based practice
* Ability to record accurate clinical notes
