Background / About Us Integrated Neighbourhood Teams (INT) is a new service that joins together providers across the community to work in an integrated way around the health and wellbeing needs of the community. It brings together a range of proactive care and support services e.g. GPs, district nurses, social services, care co-ordinators, social prescribers, pharmacists etc. to support cohorts of patients with specific health needs to help them stay well for longer. This is an exciting opportunity to play an important part in developing this new service for the patients of Church St and Newbury St Practices who have long term conditions (LTC), haven't attended for their annual health check for a year or more and have perhaps had hospital admissions for the same LTC. Main Purpose of the Job The INT Co-ordinator will work closely with the GPs, Community Services, PCN Team, Local District Councils, Social Care and Voluntary sector organisations.Provide support for GP Practices, patients, carers and professionals to improve patient care, access and navigate resources and services which will help those patient groups prioritised by the Clinical leads.Utilise Population Health Intelligence to proactively identify target group patients to improve patient care and wellbeing in the locality.Identify and agree key priorities for the local area with the Clinical Leads. Review performance of INT and benefit to patients. Ensure that the reduction of health inequalities is embedded into practices across the INT based on local population need, evidence of best practice and effectiveness. Ensure clinical staff are available for clinics noting annual leave etc. Invite Patients / Book appointments (including cancelling and rearranging when necessary). Ensure patient records are updated. Provide reception service for clinics signposting patients & making referrals as appropriate.Resolve any queries / problems / complaints. Stock Control: Order equipment / consumables and stock / restock clinical rooms.Process invoices for payment.Arrange for equipment to be PAT tested / calibrated. Develop Collaborative working and Communications. Collaborate with partners, community sector organisations and community representatives to co-produce innovative development and re-design of services. To act as meeting secretariat in the preparation of agendas and circulation of relevant papers/minutes to participants for MDTs and the INT committee. Attend to minute the meetings and ensure production of good quality minutes and dissemination of minutes within agreed timescales. Be sensitive to the local political landscape and sensitively work across organisations, often against conflicting priorities, to bring consensus enabling effective reduction of inequalities. Work collaboratively with the INT Clinical Lead(s), local health care providers and wider system partners to support development and design of INT, modelling strong collaborative behaviours that focus on improving patient outcomes and encouraging engagement.
