We have a rare opportunity to welcome a new community support worker to join our established multi-disciplinary team as we continue to grow our service.
As part of the NHS Long Term plan Lincolnshire was selected as 1 of only 12 areas to benefit from additional funding to transform community mental health services. As part of this, the Community Rehabilitation Team are finding new ways of working with people with complex trauma, psychosis, and enduring mental health needs. The team has recently extended their established Lincoln and Gainsborough service to include Grantham and the surrounding area. As part of this exciting transformation programme, we are looking to increase our established multi-disciplinary team of Community Support Workers, Occupational Therapists, Nurses, Social Workers, and Psychiatrist.
We work in partnership with housing, CMHT’s, and third sector organisations to meet the recovery needs of individuals accessing our service.
Main duties of the job
1. To work within the policies, protocols, and clinical procedures of Lincolnshire Partnership NHS Foundation Trust and to promote competent working practice within an integrated multi-disciplinary team.
2. To liaise with the Community Psychiatric Nurse / Social Worker / Occupational Therapist, and carry out assigned duties as a member of the multi-disciplinary mental health team.
3. To be prepared to work on own initiative as necessary and report to Community Psychiatric Nurse / Social Worker / Occupational Therapist.
4. Promote equality for all patients and understand their health needs.
5. Demonstrate an awareness of service user and carer perspectives on the provision of treatment and continuing care.
6. Ensure accurate record keeping in accordance with Trust policy.
7. Participate regularly in clinical supervision sessions.
8. Liaise with representatives from associated voluntary and statutory organisations and facilitate good communication processes.
9. Attend in-service training and other study / refresher days to meet individual, statutory and service needs.
10. Participate in performance reviews.
11. To work flexibly across a 7-day-a-week service.
12. Facilitate and participate in recreational and physical activities in the provision of health and social care to service users.

Detailed job description and main responsibilities
1. Evidences a high level of understanding of the effects of mental illness, alongside an empathetic and empowering approach to meet the individual needs of the patient.

Person specification
Qualifications
* Cavendish Care Certificate or willingness to work towards resourced by LPFT.
* NVQ 3 in Care or equivalent.
* NVQ 3 in Promoting Independence or equivalent or working towards achievement.
* Training in mental health awareness or other training relevant to this post.
* NVQ 2 in Care or equivalent.
Experience
* Previous relevant experience working with people with mental health problems at a nursing assistant grade or equivalent within a health and social care setting.
* Knowledge of risk assessment principles.
* Experience in providing support for people in the community.
* Experience of working with people with mental health needs.
Skills
* Effective communication with service users, other members of the team, and outside agencies.
* A patient and empathic approach. Clear and accurate clinical note keeping.
* Quick decision-making skills - calm approach and have the ability to liaise and consult with other colleagues if required.
* Plan and organise their day and ensure that their whereabouts are known.
* Ability to travel around the county independently.
