Health Care Assistant - Ashgrove Medical Group
This role is for two days per week as a HCSW within our nursing team. You will be seeing patients to undertake clinical duties such as phlebotomy, ECGs, blood pressures, diabetic foot checks and gathering information for their long term condition reviews.
Main Duties of the Job
The post holder will:
1. Be responsible for the management of a group of patients with regard to Venepuncture, Physical Assessment, Adult Immunisations, Wound Care, Minor Surgery Assisting and supporting the clinical team in Chronic Disease Management, Health Screening, Mental Health, Men's and Women's Health, and the management of Emergency Situations.
2. Be responsible for maintaining the standards of patient care set by primary care.
3. Be responsible for ensuring that all policies and procedures are observed.
4. Be responsible for the support of staff i.e. Phlebotomist, learners.
5. Be responsible for providing accurate, current, comprehensive and concise records concerning the condition of the client.
6. Liaise with the appropriate Clinical Specialists i.e. GPs, Practice teams and Nursing Team, in relation to individual patient requirements.
7. Work in accordance with the Nursing and Midwifery Council Code of Conduct.
8. Demonstrate good organisational skills with responsibility for ensuring efficient and effective use of human and financial resources.

Job Responsibilities
The range of assigned tasks will be carried out with the highest standard to support and enable registered and non-registered nursing staff to provide patient care, which reflects the quality standards in accordance with the Nursing and Midwifery Council Code of Conduct. Support the delivery of a comprehensive nursing service including chronic disease management.
The post holder is responsible for supporting the delivery of general practice nursing services to the practice population, supervised and supported by registered nurses within the practice. They will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring chronic conditions, and providing health prevention and screening activities.
They will be supervised by registered nursing staff and support training of undergraduate nurses and the wider general practice team in competencies within their scope of practice.
The role will involve flexible working hours, and working across our two sites.
Person Specification
Experience
* Substantial recent primary or community nursing experience.
* Experience of nurse-led management of long-term conditions.
* Experience of implementing protocols and clinical guidelines.
* Knowledge of health promotion strategies.
* Knowledge of local and national health policy.
* Wider health economy awareness.
* Knowledge of clinical governance issues in primary care.
Qualifications
* Level 3 HCSW Apprenticeship in Primary Care.
* Support management of long term conditions.
* Change-management skills and ability to support patients to change lifestyle.
* Communication skills, both written and verbal.
* Ability to communicate difficult messages to patients and families.
* Clinical supervision experience.
* Immunisations training (flu, shingles).
* Skills for specialist clinical conditions e.g. Spirometry, inhaler technique, peak flow, doppler assessment.
