Newcastle upon Tyne Hospitals NHS Foundation Trust are seeking to recruit a highly motivated and enthusiastic Band 6 Nurse Practitioner to work alongside colleagues within our busy intermediate care unit. Intermediate care supports people to avoid going into hospital or residential care unnecessarily, facilitating independence after a hospital stay or a period of difficulty managing at home.
Our unit provides high quality care to patients requiring rehabilitation to optimise their health before returning to their usual place of residence. We work as a multidisciplinary team with patients to achieve their goals and best outcomes.
Main duties of the job
The post holder will work as a Nurse Practitioner within the established multidisciplinary team. Responsibilities include:
* Completing patient histories and physical assessments using clinical skills.
* Participating in daily board rounds and goal planning towards discharge.
* Managing a defined caseload, including assessment, investigation, diagnosis, and treatment within defined protocols.
* Providing immediate care management and communicating plans to patients and significant others.
* Liaising with other healthcare professionals as necessary.
Person specification
Qualifications & Education
* Registered Nurse (appropriate to branch)
* Current NMC registration
* Diploma plus specialist qualifications/experience; prepared to undertake degree level study
* Working towards Nurse Practitioner Qualification or equivalent
* Clinical Skills Course
* Non-Medical prescribing if relevant to role
Knowledge & Experience
* Extended clinical practice underpinned by training and assessment (e.g., venepuncture, cannulation skills)
* Evidence of ongoing continuous professional development
* Understanding of relevant NHS policy
Skills & Abilities
* Able to work independently and as part of a multi-disciplinary team
* Effective organisation and time management skills
