The successful applicant requires an experienced and flexible hands-on approach, someone who will subject to highly confidential information and who will be able to maintain total confidentiality. This role will report directly to the Senior Occupational Therapist. Job purpose To actively seek to empower service users to gain and maintain control over all aspects of their lives, by utilising a range of assessment skills to highlight their strengths, abilities and skills and seek to maximise the development of these. To plan and implement therapeutic intervention programmes to include specialist equipment, environmental changes, visual prompts and skill development. To support and promote the Implementation of Active Support throughout the organisation. Duties and role Clinical Responsibilities To carry out detailed assessment of referred residents using a range of standardised and non-standardised assessments in order to establish strengths abilities and areas of difficulty within the domains of self-care, productivity and leisure. To plan and implement intervention programmes for needs identified following assessment in order to improve occupational performance. To support the implementation of Active Support through assessment of individuals support needs and advice and guidance to staff To research appropriate equipment, adaptations, visual resources and activities relevant to an individual s needs following assessment To carry out risk assessments relating to individuals and activities, and to ensure that all risk assessment proposals are carried out. To advise, organise and co-ordinate with the support staff on the implementation of structured activities for the service users throughout the company homes. To support the Assessment and Referral Process as required. To monitor and evaluate interventions in order to measure progress and ensure the effectiveness of the interventions Communication and Documentation To form professional relationships with clients and their families, communicating with them in a way that they understand and that respects their views, autonomy and culture. To complete therapy progress notes and recording forms for all interventions completed within established record keeping and confidentiality policies. To provide detailed reports including costs for equipment/adaptations To provide detailed support plans for care staff to ensure therapeutic recommendations are carried out in a consistent and appropriate manner. To attend multi-disciplinary case conferences to plan and review ongoing interventions. To report to the home manager and line manager any recommendations which have not been carried out. To liaise with a wide variety of other professionals including doctors, physiotherapists, social workers, equipment suppliers and service users families. Education and Training To review and reflect on your own practice and performance through regular participation in professional supervision and appraisal To undertake relevant activities to meet training objectives identified with your supervisor. To attend mandatory training courses, on or off site as and when required. To contribute to training of support staff related to individual client need or to support implementation of Active Support. General To take the lead for particular clinical or professional projects as directed by senior management. To interact in a professional manner at all times. To take responsibility for Health & Safety issues, eg. reporting any dangers to self and others, etc. To report any changes in resident's condition to the Manager or Senior Carer on duty. To care for residents' property and that belonging to the Home as directed. To care for equipment and report any defects to the Manager. To adhere to all aspects relating to confidentiality and not discuss the Company, service users, or other information which is acquired via the job, with others not connected to or who have no right to the information. To contribute positively to meetings such as supervision, appraisal, team meetings and training events. To work in line with all the companies policies and procedures. To exercise good personal time management, punctuality and consistent reliable attendance. To be flexible and work to the benefit of service users, and to assist with any other duties if required.
