Job summary Surrey Lodge Group Practice is a 2-partner practice with an interesting & diverse population of around 10000 patients. We are developing our motivated and supportive team and are looking to recruit a Practice Nurse. We would welcome a Nurse at any stage of their career. We have a very experienced clinical team with 10 GPs, Pharmacist, ANPs, Nurses and HCAs. We also have a very supportive and friendly administrative team. This role will include cytology, asthma and COPD management, diabetes management, childhood/adult Immunisations, requesting Pathology Tests, for example Urine Culture, Swabs, and Chronic Disease Management and evaluation of care. Phlebotomy Services as required Home Visits where required Provide timely Clinical and Culturally appropriate Health Services through: Ongoing Health Education and advice, including counselling to improve health and prevent disease. Use of recall and reminder systems and, as appropriate, referral to National Screening Programmes. Education to assist patients to reduce or change risky and harmful lifestyle behaviour.
