Job summary The role of the Integrated Care Home Co-ordinator is to deliver high quality, caring and compassionate patient care. The post holder will undertake assessment, implementation, and evaluation of patient care needs for a defined group of patients under the indirect supervision of a registered professional in their specialist area. The post holder is required to work as part of a team committed to delivering and improving a high standard of nursing care within UHCW. Take a lead role in navigating referrals from healthcare professionals to the appropriate service, working autonomously and prioritising workload according to the demands of the service. Work collaboratively with multidisciplinary teams and agencies, which may include health and social care services to support navigation of patient referrals into services which best meet their needs.
