Role Purpose
An opportunity has arisen for a social worker to join a busy Adult social work and practice Team based in the
Berwickshire Office in Duns for a period of 6 months.
The Team is made up of Social Workers, Nurses, Occupational Therapists and para-professionals.
The team provide a Community Social Work service for the local
community of Duns, Eyemouth and the surrounding villages.
The successful post holder will at times be required to provide duty cover for the team - responding to telephone calls, office callers and unplanned contacts with other professionals.
The post holder will assess needs, plan for and review support which is put in place. Although predominantly community based, there may be times where you are required to complete hospital discharge assessments.
If suitably
qualified to do so, they will participate in Council Officer duties, and will engage in all other aspects of the
Team work - including attendance at Team Meeting and stakeholder meetings with Health colleagues.
The post holder will have access to regular supervision from their line manager and peer support from the friendly and knowledgeable team members.
Principal Duties & Responsibilities
• To undertake social work assessments, identify options, make recommendations, and
effective solutions; and to provide specialist advice guidance and support as appropriate to individuals and carers.
• To complete risk assessments and support plans based on the outcomes of the assessment
• Identify, recommend and evaluate appropriate resources to meet eligible assessed need
• To refer to other professionals where required or other partnership agencies including third sector groups.
• To participate in formal supervision sessions as required and to make critical analysis of personal
practice and performance in conjunction with line management.
• To be responsible for a caseload and the recoding of all work undertaken in accordance with
Recruitment Profile
Scottish Borders Health and Social Care partnership policies and legislative obligations.
• To regularly meet and contribute to one’s own professional supervision and continuous professional
development.
• To participate in such training programmes as may be deemed appropriate, including the supervision
of students; and to act as a resource for other professionals, care groups, units and departments
• To carry out any other duties within the scope and intention of the job purpose as determined by the
line manager.
Essential
Qualified Social Worker (degree/diploma in Social Work)
Registration with the Scottish Social Services Council
A current driving licence and use of a vehicle is an essential requirement of this post.
