Key responsibilities: Identify service needs and employ advanced communication, negotiation, and collaboration with various stakeholders, including senior managers, commissioners, primary care, general adult, forensic and substance use services, prisons, probation services, and other independent providers as deemed appropriate, to create flexible, integrated, and comprehensive services. Collaborate closely with service users to explore the aforementioned issues, aiming to enhance understanding, insight, and motivation, emphasizing self-management of risk as an integral aspect of the recovery process. Work in partnership with service users to recognize early signs of mental state and behavioral deterioration, as well as potential triggering factors that may contribute to recurring risk behaviors. Assist service users in developing skills for self-managing their medication through structured self-medication programs. Support users in reconnecting with family and friends, promoting engagement or re-engagement. Assess situations by gathering information from patients, their families, and other pertinent sources, considering the patients' strengths, limitations, risks, and vulnerabilities.
