Interim Adults Social Worker - Hospital Discharge Team

Pay Rate: £22.69 per hour PAYE
Working Hours: 37 Monday – Friday, 08:45 - 17:15
Assignment Duration: This opening assignment is for 5-6 months

Hybrid working
Experienced social worker with a mental health background to join the mental health hospital discharge team in the City Council and across the Trust work sites.
Key Responsibilities:
1. Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge
2. Some mental capacity assessments, BI decisions and safeguarding as required
3. Attending 117 meetings, ward rounds, professional meetings
4. Facilitating learning opportunities for ASYEs through shadowing
5. Participation in duty rota
Qualifications:
1. CQSW, CSS, DIPSW Social Work Degree or equivalent
2. Registration with Social Work England
Knowledge:
1. Knowledge and application of Social Work Interventions
2. Knowledge and ability to undertake appropriate investigations under safeguarding procedures
3. Knowledge and application of relevant legislation, policies and guidance
4. Knowledge of the referral and signposting processes regarding Housing, Welfare Benefits and other community support services
5. Knowledge of and adherence to local and national policy affecting adult social care
6. Basic knowledge of equipment and adaptations available to promote independence e.g. assistive technology
7. Practical understanding of care and support needs of people as a result of a physical and/or a mental condition
8. Understanding of person-centred practice and skills relevant to this role
Experience:
1. Social Work experience of working with people and the public, voluntary or independent sector
2. Experience of multi-disciplinary working
Skills & Abilities:
1. Effective negotiation skills
2. Excellent interpersonal and communication skills, both written and verbal at an appropriate level
3. Ability to be creative and innovative in relation to support networks
4. Ability to use own initiative, adapt and respond to dynamic situations. Able to self-manage, self-motivate and be accountable for own work
5. Ability to travel city wide to fulfil the role
6. Able to work with minimum supervision and make timely decisions to maintain the integrity of data
7. Ability to assess needs of people and identify and mitigate risk with skills in assessment and individual care and support planning with people and carers
8. Ability to create individual plans, understanding responsibilities and tasks associated with developing networks and/or arranging services in accordance with their eligible needs
9. Ability to work positively as a member of a team, and to liaise with other colleagues/professionals within the context of the person’s own support networks
10. Proficiency with spreadsheets and data analysis
