*Work within the New Forest Locality of the North and West Hampshire Community Integrated Respiratory Service, while being aware of the work in the other localities and being prepared to assist in covering other localities in order to maintain service provision across the region. *Each locality will have direct working relationships with the acute Trust at its centre working closely with the respiratory consultants from the Trust. *Supervise the Band 2 Clinical Assistants in your locality. *Be involved in the planning and delivery of patient education sessions *To assist the team with implementation of programmes of evidenced based, care for respiratory patients within North and West Hampshire, ensuring that interventions meet best practice guidance. *To offer a range of interventions including self management plans, medicines management, chest clearance, comprehensive oxygen assessment and ordering, pulmonary rehabilitation and end of life care. *Plan and manage own workload, undertaking individual patient assessment including Capillary Blood Gases (CBGs) where indicated. *Contribute to the ongoing development of pulmonary rehabilitation and home oxygen assessment, fostering a multi-disciplinary approach, with best evidence-based practice. *Ensure referral to psychological therapies is offered to patients when indicated. *Be aware of when a patient is approaching end of life and ensure the pathways are followed to provide holistic, comprehensive support to the patient through this time. Referral to hospice and psychological therapies as appropriate and take a role in preparing and supporting patients through this process. *Challenge current practice, ensuring it is evidence based and reflects current national guidelines. *To ensure relevant clinical governance issues are identified and addressed *Work and liaise with primary care, community services, third sector services and all other relevant care givers to provide a seamless service for the patient. *Forge links with charities associated with respiratory conditions and promote a greater understanding of respiratory conditions to the general public *Contribute to the development and evaluation of policies, guidelines and protocols relating to all elements of the service *Ensure information and data are recorded to fulfil requirements for KPI reports *Collect, collate and present data that evidences the benefits of the service and supports future service developments. *Identify opportunities for research and development activity.
