Your Main Responsibilities:
* Specialised Care: Provide advanced nursing assessments, plan and implement care for patients with tissue viability needs, and evaluate outcomes.
* Prevention and Treatment Expertise: Work with patients and professionals to manage and treat pressure ulcers and leg ulcers, utilizing evidence-based practices.
* Leadership and Collaboration: Take initiative, make independent decisions, and work collaboratively with colleagues across disciplines to deliver holistic patient care.
* Continuous Improvement: Participate in quality improvement initiatives, audits, and service development projects to ensure our care remains at the highest standard.
Person Specification:
* Essential Qualifications: Registered Nurse with experience in leg ulcer management or tissue viability.
* Teaching and Training: Ability to work with and lead others across professions.
* Community nursing qualification and experience of working in primary care.
* Desirable: Experience in teaching, leading teams, and community nursing qualifications.
* Skills & Knowledge: Advanced wound care skills, the ability to make informed decisions, strong organizational skills, and effective communication. Familiarity with NHS policies, quality improvement, and clinical governance is desirable.
* Tissue Viability CPD.
