Community Support Worker - Assertive Outreach

An exciting opportunity has arisen within our Community Mental Health Service to work within our Assertive Outreach Team, based in Bridgwater. We are looking for an enthusiastic, caring person with experience of working with people who are suffering serious mental ill-health.
Our team responds rapidly to the needs of adults needing mental health advice and support. The team is established, supportive, knowledgeable and above all passionate about what they do!
This opportunity is for 5 days per week (37.5 hours), 9am-5pm - we can be flexible around condensed hours and start/finish times.
We are a supportive, compassionate multi-disciplinary service where every person is an integral part of the team.
Main duties of the job
As a Community Support Worker (AOT), you will play a vital role within the Assertive Outreach Team, providing care and support to an allocated group of service users with complex needs, across a shared team caseload. You will support service users in their community environment and also those who are in-patients in supporting their return home, to promote their recovery. You will assist the professionally qualified team members to assess, plan, implement and evaluate care plans.

Person Specification
Qualifications
* NVQ 3 Promoting Independence or equivalent Apprenticeship, or willing to undertake full training to achieve this standard.
Experience
* Experience of working with people in a Secondary Mental Health setting.
* Experience of working in community settings.
* Understanding of Serious Mental Illness (SMI).
Additional Criteria
* Creativity, flexibility, enthusiasm and the resilience to tolerate a range of challenging situations.
* Ability to work independently in lone working situations.
* Good written and verbal communication skills.
* A good team worker.
* Access to own private transport and willing to transport clients as required.
