Job summary An opportunity has arisen for a part time specialist nurse in the remote monitoring team. This role will be working in the small remote monitoring team. The remote monitoring team is a new service provided by Dorset Health Care. The service uses a digital platform to monitor patients in their own place of residence, this enables patients to leave hospital earlier or prevent them from being admitted. The aim of the service is provide remote monitoring to different specialist pathways working with health providers across Dorset. The remote monitoring team provides a service 7 days a week from 08:30 to 18:00 at present but the service is commissioned to provide an 08:00 to 20:00 service once it is full established. The applicate will be expected to work week ends on a roster bases. The hours of working can be negotiated at interview to fit in with the needs of the service. The post is advertised for someone to work 30 hours per week but applicants wanting to work less hours will also be considered. The applicant must have good clinical and communication skills in assessing patients. They must be able to be reassuring for patient using remote monitoring. Attend integrated MDT meetings as part of the clinical team caring for remote monitored patients. The post holder will be expected to work the within the to proved clinical remote monitoring care to patients with variety of clinical needs and work with other clinical specialist across Dorset to develop the service. Main duties of the job Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position. You will need a sound understanding of the current primary care agenda, including the management of long term conditions also with excellent communication skills and to be able to demonstrate effective leadership, decision making skills and resilience. You will provide high quality clinical care, management of staff and case management for patients in the community setting. You must be able to demonstrate a high level of clinical knowledge, team working, good interpersonal skills and time management. You should have experience in caseload management and be able to priorities work. The role is diverse, challenging and extremely rewarding.
