Job responsibilities This list is not exhaustive and will flex and change to meet the demands of the practice, and with the successful candidate depending on what skills they currently hold, and what they are willing to work towards: Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health Implement and evaluate individual treatment plans, manage chronic disease registers and run clinics for chronic disease patients Provide daily routine nursing care to patients as required in accordance with clinical-based evidence Provide wound care and dressings for patients Assist the Nurse Practitioners and Clinical team as and when necessary within the scope of practice nursing Maintain accurate clinical records Ensure records and consultations are coded and recorded effectively and accurately Chaperone patients where necessary Assist GPs with minor surgery, coil and nexplanon clinics when required Support the team in dealing with clinical emergencies Recognise, assess and refer patients presenting with mental health needs Implement and be active in running vaccination programmes for adults and children Delegate clinical responsibilities appropriately and provide leadership and mentorship to trainees and junior staff members Support the clinical team with all safeguarding matters, in accordance with local and national policies Deliver opportunistic health promotion where appropriate Practice nursing duties including venepuncture, BP, ECG, Ear Irrigation, injections and health promotion clinics Administrative duties including ordering, stock control and keeping rooms stocked with equipment. Undertake visits to our housebound patients or those in or Learning Disability nursing or residential care, e.g. to administer Flu or COVID vaccines.
