An opportunity for a 6 month secondment post has arisen for a motivated, dynamic person to join our senior team in our Older Persons Rapid Access Clinic and Frailty Unit at the Ealing Hospital site.

You will need to be able to support the junior team as well as the senior team and take charge of the unit in the absence of the ward manager.
OPRAC /Frailty is part of the Integrated Medicine division. We care for patients from GP and Hospital referrals. We manage patients with Parkinson's disease, Heart Failure, Anaemia, Osteoporosis and various other medical symptoms.
We pride ourselves on maintaining high standards of care as well as creating an environment for professional development. The team works collaboratively with Consultants, Medical teams, Specialist Nurses and Pharmacists to deliver individualized patient-focused care.
Please note- the working pattern for this role is Monday to Friday between 8.00 am to 6 pm.
Main duties of the job
* OPRAC /Frailty is based within the frailty unit and the successful person will be expected to work with the multidisciplinary team to provide early and proactive Comprehensive Geriatric Assessment (CGA) to patients who meet the referral criteria when the service requires. The holistic assessment will include clinical, social, functional, emotional, and cognitive elements as documented within the CGA.

Person Specification
* 2 years post graduate experience preferably in a similar role
* NMC registered
* Experience in working in a similar role
* Evidence of post graduate study in related clinical area

Salary: £42,939 to £50,697 per annum inclusive of HCAS
