Main duties of the job As a key member of the multidisciplinary Home First team:Gather information concerning patients' current medication/medical historyProvide in-reach to the hospital to support discharges to communityCarry out domiciliary visits to patients where appropriateProvide assessment of patients' ability to manage their own medicinesProduce MAR charts to support medicine administration by Home First care staffProvide medicine-related advice to patients, families and carersLiaise with other service providers to improve the transfer of care between care settingsParticipate in MDT meetings and participate in service developmentCarry out clinical audit collection.The successful candidate must have access to a car and hold a valid driver's license for work purposes.
