We have a dynamic team of nurses, healthcare assistants and phlebotomist providing chronic disease management and enhanced services to our 14,500 patient list. The ideal candidate will have proven experience in a primary care setting. We are looking for an experienced primary care Nurse who has at least Chronic Disease Management in either Diabetes or Asthma. Please see the job specification for further details of requirements. You should be keen to keep yourself updated in the new challenges facing primary care and be prepared to undertake further training as required for the position. Main duties of the job To provide and maintain a high standard of nursing care for patients, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The duties will include but not limited to day to day primary care nursing such as phlebotomy, ECGs, wound care, adult and child immunisations, B12 injections, BP monitoring as well as Enhanced Practice Nurse skills in chronic disease management, cervical screening and compression dressings.
