Job Description:
The North Lincolnshire Home First Community Rehabilitation service is a registered Rehabilitation and Reablement service that provides time-limited intervention and therapies to people within their own homes. We strive to deliver support that puts people at the centre of our services. Our aim is to ensure that a person can remain at home and feel confident and safe to live as independently as possible.
Reablement assistant post holders will form part of our expanding transitions team that will work to ensure smooth and effective transition from the hospital with protected/dedicated visits available to support discharge from SDEC and A&E in a timely manner. Reablement workers also deliver extensive care and support to service users within the community whilst optimum fitness is reached to enable participation within the rehabilitation program as well as supporting individuals who have been identified as requiring long-term care.
As part of the reablement role, the successful candidate will provide rehabilitation and reablement support to individuals following an accident, incident, or illness allowing a return to independence. This involves following therapy plans and working in conjunction with other health and social care professionals.
The role also provides a crisis response to support when unforeseen events occur such as falls and carer breakdown.
The role involves direct work such as supporting with moving and handling, attending to personal care needs, and administering medication as well as maximising opportunities for social inclusion and emotional wellbeing by signposting to external organisations and professionals.
This role offers a fantastic work-life balance through a 25-hour per week contract. This operates on a rolling rota basis, requiring working 3 to 4 shifts per week, with various hours on a rota pattern between the hours of 07:00 am to 23:00 pm. For example, shifts may be from 07:00 to 13:35 or from 15:55 to 22:30. For those that would like additional hours, there is an opportunity for working overtime, which when working weekends includes additional weekend enhancements. Earning potential in this Grade 3 position is currently £24,027 to £24,790 pro rata for part-time hours.
