Senior Occupational Therapist (Stroke Early Supported Discharge Team)

We are seeking a motivated and skilled Band 6 Occupational Therapist to join our multidisciplinary community-based stroke service. You will play a key role in delivering person-centred, goal-oriented rehabilitation to stroke survivors in their own homes or community settings. You need experience in stroke rehabilitation or neurorehabilitation (community experience desirable) and the ability to get to the patient's homes.

Main duties of the job
The Stroke Early Supported Discharge team covers an area of East Berkshire, Surrey and Hampshire. Providing intensive stroke rehabilitation to those patients discharged from the acute wards but also from stroke clinics and emergency care.
Responsibilities include clinical management of patients and completion of relevant paperwork. To contribute to the continued development and maintenance of efficiency and safety in the Stroke ESD team. To be flexible at times of increased capacity to the service. The Senior OT contributes to the provision, co-ordination, and development of OT services with the OT Team Leaders. Assists in training and supervising of other staff and students and participates in administrative and clinical functions of the Stroke ESD team as a whole.

Qualification
* Degree or Diploma in OT
* HCPC registration
* Clinical educator's course
* Advanced study in stroke
Skills & Knowledge
* Good organisational skills
* Effective written and oral communication skills
* Team player - understanding team dynamics

* Ability to reflect and critically appraise own performance
* Training and facilitation skills
* Use of an electronic patient record
Experience
* Broad knowledge of evidence-based practice
* Worked in some of the following areas: acute medicine, Orthopaedics, surgery, elderly care, A&E, neurology, community rehab
* Experience in treating stroke patients
* Broad knowledge of health legislation in current practice and clinical governance
* Experience of research and/or audit
* Supervision to support staff and/or students
* Worked in community-based service
Special Requirements
* Must be able to travel between patient visits in the community
