Job summary
An exciting opportunity has arisen for a qualified Advanced Clinical Practitioner to join our Acute Mental Health wards at the Redwoods Centre. This post will cover Pine ward, Birch ward and our improved 136 suite. Each acute ward is 16 bedded mixed sex accommodation and our 136 suite has 3 beds (2 adult and 1 CYP). The successful applicant will join a dynamic and clinically well-established team, with leadership qualities. The successful applicant will have the opportunity to liaise directly with a variety of other professionals and agencies to gather the necessary information and resources to support their collaborative decision making with patients who are suffering with mental health difficulties.
Pine and Birch are Acute mixed sex inpatient wards that are 16 bedded, each ward has a full Multi-Disciplinary Team which includes a range of different professionals which includes Consultant Psychiatrists, Medical, Nursing and Occupational Therapy staff plus a wide range of support services.
We have recently undergone expansion of the Health Based Place of Safety (HBPOS) at the Redwoods Centre. The new place of safety (136 suite) consists of a 3 bedded unit, with secure outdoor and assessment space.
In addition, the successful applicant will hold excellent verbal communication skills and can support other agencies with referral options and clinical decision making.
Main duties of the job
1. The direct provision of high-quality evidence based person-centred physical and mental health care to inpatients within the acute inpatient ward.
2. Providing leadership in the delivery of high quality, safe and effective care to the wider multi-disciplinary team.
3. Receive patients with undifferentiated and undiagnosed problems and assess their health care needs, based on highly developed nursing knowledge and skills, including physical examination skills.
4. Plan and coordinate the physical healthcare needs of inpatients on the ward; including the requirement for physical examinations and investigations.
5. Order necessary investigations, and provide treatment and care both individually, as part of a team, and through liaison with / referral to other agencies, ECGs, Clozaril Bloods.
6. Plan, develop and manage specialist packages of care, employing the Care Programme Approach (CPA), working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use.
7. To provide clinical leadership and supervision to trainee ACP's, trainee NMP's and registered nursing staff if required.
Facilitating learning through the development and delivery of specialist clinically focussed education and training programmes.

21. Draw on a diverse range of knowledge in making decisions to determine evidence-based therapeutic interventions (which will usually include prescribing medication and actively monitoring the effectiveness of therapeutic psychological interventions).
22. Use professional judgement in managing complex and unpredictable care events and capture the learning from these experiences to improve patient care and service delivery.
23. Undertake weekly assessments of all inpatients under their care, addressing issues and assessing and managing risks identified.
24. Prepare reports encompassing medical and advanced nurse practitioner interventions in readiness for CPA meetings.

26. Independent and supplementary prescribing- practicing within local and national policy and guidance.
27. Undertake health promotion and prevention activities including, for example, educating patients in the side effects of medication and lifestyle choices.
28. Develop individualised discharge summaries on behalf of the Responsible Clinician.
29. Identify, minimise and report risks of danger, harm or abuse.
30. Responsible for documenting plans of care and for ensuring that plans remain current and ensuring high standards of record keeping.
