Clacton PCN are seeking a PCN lower limb nurse to join our team for a 1 year fixed term contract (with potential for extension). This position is offered on a part-time basis for 22.5 hours per week, over 3 days - some Saturday work may be required. As a skilled nurse, you will provide specialist, evidence-based care for patients with lower limb conditions such as leg ulcers and complex wounds. Responsibilities will include; Perform holistic nursing assessments, developing individualised care plans and offering ongoing evaluation. Deliver direct patient care, including prevention and management of acute or complex wounds. Collaborate with PCN colleagues to develop and refine our Lower Limb Therapy Service and Well Leg Service. Mentor and educate healthcare professionals, patients, and carers on best practices for leg ulcer management and wound care. Participate in clinical audits and research activities to drive continuous improvement in patient outcomes. Conduct thorough assessments, provide specialist advice, and ensure high-quality nursing care. Oversee the lower limb caseload, contribute to strategic planning, and act as a role model for professional practice. Lead audits, analyse data, and integrate evidence-based findings to enhance service delivery. Provide training and guidance to healthcare teams, ensuring consistent, high standards of care. Maintain accurate, up-to-date patient records, adhere to infection control standards, and follow organisational policies. About the Candidate We are looking for a proactive, compassionate registered nurse with a passion for improving patient outcomes, particularly in leg ulcer and lower limb care. You will need: Current NMC registration and at least 3 years’ post-qualification experience. Experience in primary care or community nursing (especially wound care, leg ulcer management). A solid understanding of relevant clinical guidelines and best practices for lower limb conditions. Excellent communication, leadership, and team-working skills. A willingness to work flexibly, respond to the evolving demands of a busy PCN, and travel between local GP practices. A post-registration qualification or specialist training in leg ulcer management or tissue viability, as well as experience mentoring or teaching healthcare professionals, is highly desirable.
