We are seeking a highly experienced registered nurse, who is capable of working autonomously within a multidisciplinary palliative care team, to promote and maintain the quality of life for patients with complex symptoms and needs, living with life-limiting conditions within a community setting. The successful candidate will also provide operational and clinical leadership to the community specialist palliative care nursing team.
The proposed interview date is: 18th March
Responsibilities include:
1. Identifying, organising, and evaluating specialist intervention in response to complex needs on a daily basis, including oncological/medical/palliative care emergencies.
2. Analysing clinical options and liaising with appropriate members of the multidisciplinary team.
3. Supporting other team members in clinical decision-making, while demonstrating professional and leadership skills in managing a busy service.
4. Contributing to the allocation and proactive review of all referrals within the service.
5. Supporting and directing timely intervention for patient review and assessment.
6. Allocating referrals to an appropriate team member.
7. Working autonomously to assess patients and carers within the community with complex physical, emotional, social, and psychological symptoms on a daily basis to enable an appropriate plan of care to be developed.
