Description

Today, CADES operates over 30 community living arrangements, adult day programs, the George Crothers Memorial School, and early-intervention services.
CADES seeks a Program Specialist to further our mission of empowering those with special needs to achieve their highest potential.
Position Summary: CADES maintains residential facilities in Delaware, Chester, and Bucks County. The Program Specialist will oversee and ensure the completion of all required program planning for individuals.
Essential Duties and Responsibilities:
1. Completion & oversight of implementation of all program planning within the guidelines of regulatory standards including assessments, ISPs, monthlies, quarterlies, plans of support.
2. Role model effective communication & problem-solving skills.
3. Monitor all needed DME for individuals including ensuring equipment listed in ISP is present & functioning; training material is available in the home; equipment is clean & well maintained; requests for repairs & new equipment are submitted & follow-up is completed.
4. Point person for Admissions Coordinator throughout the referral process.
5. Oversee & manage regulatory monitoring for individuals.
6. Liaison for communicating and updating the team on the needs of the individuals.
7. In conjunction with Program Manager, act as the liaison for Health Care Coordinator, Operations Manager, Trainer & Administrative Coordinator for all homes, individuals & staff under the assigned area of oversight.
8. Manage individual financials ensuring completion of spend downs, timely completion of contracts, timely payments of bills and monthly account audits.
9. Monitor & oversee ISP implementation within the home.
10. Review and analyze documentation to ensure compliance.
11. Complete audits & ensure compliance of the health and safety of individual clients, required documentation, physical plant, vehicle maintenance, and staff safety requirements.
12. Monitor and oversee medication administration.
Qualifications:
1. Bachelor’s degree with 2 year’s experience working directly with individuals with disabilities or Associate’s degree (or equivalent) with 4 year’s experience working directly with individuals with disabilities.
2. Valid driver’s license and adequate 3-year driving record.
This position requires the ability to perform the essential functions of the job, including the ability to lift a minimum of 20 pounds, bend, twist, and turn.
