We are looking to recruit a registered nurse who has an interestin primary care nursing.
This is an exciting opportunity to join a forward thinkingpractice, experience is desirable but not essential.
Our practice is based in Rhyl, North Wales and covers a verydiverse population of 17,000 patients making the role interesting and varied.We have a friendly, and supportive clinical team, we also have a forward-thinking management and administrative team.
Main duties of the job
We are looking for an enthusiastic and compassionate nurse who wants to be part of our team.
All successful candidates should prioritise the delivery of excellent patient care as well as having a passion for learning, self-development and maintaining the standards and reputation of the practice.
The role will involve working within the clinical team, the ideal candidate would have experience of practice nursing and the necessary skills including the following
Wound care
Nursing injections
Childhood/Adult Immunisation
Anticoagulation monitoring
Cytology screening
Management of long term conditions
The registered nurse would be expected to deliver home visits if required/necessary for example, home flu vaccination visits
Appropriate training will be provided for the successful candidate/s

The main Practice site is a large purpose-built Primary Care Facility which provides the team with a contemporary working environment. we have a smaller branch facility. Staff rotate over both sites
Job description
Job responsibilities
1. Assist the practice develop and deliver specialist primary nurse led services including promoting healthy behaviours
2. Maintain disease registers in liaison with administrative staff.
3. Undertake annual reviews of patients.
4. Administer child and adult immunisations and vaccinations in accordance with national and local programmes
5. Competence in anaphylaxis and resuscitation techniques.
6. Ensuring safe storage, rotation and disposals of vaccines and drugs and that adequate stock levels are maintained
7. Be able to undertake procedures including: BP management, cytology, wound care, nursing care injections
8. Promote health and wellbeing, giving general education advice on diet, smoking chronic disease management, exercise.
9. Initiate and carry out health screening as agreed with the practice clinical team
10. work with people with long term conditions in order to support compliance with prescribed treatments
11. Provide support and encourage individuals, families and groups to address issues which affect their health and social wellbeing
12. Ensure all data protection requirements are met when gathering, recording and storing patient data
13. Liaise and maintain good working relationships with all members of the practice team.
14. Work within the multi-disciplinary team within the practice and across the wider health system
