Job summary We are looking for dynamic, passionate and motivated nurse with experience of Community Nursing to be a Team Leader within the Falmouth/Penryn Community Nursing Team. The post holder will work with the team to ensure provision of a holistic nursing assessment and high quality, safe, evidence based care to patients and carers with the aim of maintaining patients in their own homes. Main duties of the job As an autonomous practitioner the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. They will manage a clinical caseload within a community nursing team with guidance from the Community Nurse Team Leads, Community Matrons and Integrated Care Team Leads. Ensuring provision of a holistic nursing assessment and high quality, safe, evidence based care to patients and carers with the aim of maintaining patients in their own homes. This includes physical examination and detailed history taking, diagnosis of deterioration in pre-existing conditions and treatment planning. The post holder will work in this adaptable and evolving role, focusing on safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings. Implementing personalised care plans and collaborating with the whole community resource to facilitate ongoingcare management. To work in partnership with patients and carers and members of the multi-disciplinary team to offer support and advice and promote informed choice; maximising independence and optimising quality of life. This post holder will be required to provide clinical management and leadership to all staff within the team.
