Main duties of the job
1. Support the Dementia Outreach Service with the implementation of patient care plans, support and advice for care homes in the Broxtowe area.
2. Monitor and record individuals' mental health status and report any significant changes.
3. Assist individuals in developing and maintaining coping strategies to manage their symptoms.
4. See patients who are permanent residents in Care Homes within the Broxtowe area.
5. Carry out duties which involve making and receiving phone calls from care home staff offering support and guidance, reviewing their electronic records, and making entries.
6. Attend meetings face to face and on MS Teams.
7. Be a member of a multidisciplinary Team.
Job responsibilities
This is an exciting opportunity to be part of a specialist Outreach service for clients with dementia in residential/nursing care providing specialist nursing/healthcare advice, treatment and teaching/education as appropriate under the guidance of professionally registered team members. To contribute to assessment and interventions carried out by the dementia outreach and to engage with the Enhanced Health in Care Homes Service (EHCH) and with GPs in the local area.
Part of a multi-disciplinary Outreach service which works with both Independent Sector and Statutory Sector providers of Residential Care to Older People with dementia. To increase the capacity, confidence, competence and willingness of those organisations to provide the right level of care to this vulnerable client group and by doing so to increase standards of care, improve quality of life, increase independence and choice. You will be supported through training and supervision.
As this role is community-based covering the Broxtowe area, a full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Qualifications
* NVQ level 3 Health and Social Care Certificate or equivalent experience
Knowledge
* Basic understanding of mental health conditions
* Understanding and caring approach to patients
* Understanding of dementia
Skills
* Ability to work as part of a team
* Diplomatic, caring and sympathetic
* Develop observation skills with the ability to report and record physical/mental health changes
Experience
* Experience of working in a Healthcare setting with older people who experience mental health issues
* Experience of working in a Healthcare setting with people living with dementia
