Who You Are The ideal candidate is an experienced adult mental health social worker ready to join a fast-paced and integrated team within the Neighbourhood 3 Primary Care Mental Health Team (PCMHT) at Lewisham Council. You will have experience of adult casework in community mental health settings, a strong understanding of clinical and social issues related to mental health, and the ability to manage risks and safeguard adults. You are committed to the ethical values of service delivery and personalized social care and possess the ability to work effectively under pressure. What the Job Involves This role involves acting as a Mental Health Practitioner with responsibilities for providing care coordination for adults with mental illness. You will screen new referrals, carry out strength-based assessments, and lead on safeguarding and Care Act assessments within a multidisciplinary team. Duties include engaging in rota responsibilities, developing support plans, stabilizing service-users, and working within various legal frameworks like the Care Act 2014, Mental Health Act 1983, and Mental Capacity Act 2005. You will foster joint working with colleagues from South London and Maudsley NHS Foundation and the voluntary sector to provide seamless health and social care services. Skills Experience in adult mental health casework Strong assessment and engagement skills Knowledge of risk management in mental health Understanding of statutory frameworks: CA 2014, MHA 1983, MCA 2005 Adept at managing caseload in integrated multidisciplinary services Proficiency in IT systems like EPJS and report writing Good communication and interpersonal skills
