Gillingham Medical Practice are looking for a highly motivated Frailty Practitioner to work within the Practice alongside our existing Advanced Nurse Practitioner and GPs.
You will be required to work 25-30 hours per week.
Main duties of the job
You will be responsible for assessment and co-ordination of care and services to people with multiple co-morbidities and complex needs who live in their own homes and care homes.
You will work autonomously, assessing individual patient needs, initiating investigations, with support determining a differential diagnosis and initiating appropriate holistic, evidence-based treatment and care.
You will refer patients who require medical intervention to the appropriate services.

As a Frailty Practitioner, you will be an experienced Registered Practitioner, who preferably has experience and knowledge around the care of older people and with patients with frailty and complex health needs.

Job responsibilities
1. To coordinate the delivery and development of the specialist service through the use of effective evidence-based practice in line with agreed national standards for patient safety and monitoring.
2. To be accountable for the planning and delivery of safe, effective, and compassionate patient care in relation to dedicated pathways of care, working as an autonomous practitioner.
3. To work autonomously, assessing individual patient needs, initiating investigations, with support determining a differential diagnosis and initiating appropriate holistic, evidence-based treatment and care. Refer patients who require medical intervention to the appropriate services.
4. Manage care at the interface for patients in their own homes and Care Homes, preventing avoidable admissions.
5. To continuously improve clinical care standards in line with the clinical governance framework.
6. To exercise accountability as set out in the NMC Code.
7. Provide specialist clinical advice and support to the patients and clinical staff on the management and care of patients.
8. To provide direct access for patients, either face-to-face or telephone consultation. Along with telephone triage where appropriate.
9. To support GPs and ANPs in care home ward rounds, to review patients and play a key role in optimising complex treatment regimes and preventing avoidable hospital admissions.
10. To ensure effective communication internally and externally regarding patient care and resources.
11. To play a key role in ensuring that national and local standards/targets for the specialist area are met.
12. To comply with the practice's vision, values, and behavioural compact.
13. Deliver public health initiatives as prioritised by service lead.
14. Ensure effective communication with patients, relatives, and carers, taking into consideration level of understanding, culture, and background.
15. Escalate any concerns to the Clinical Supervisor.
16. Encourage relatives to take an active interest in the welfare and care of the patient.
17. Respond to informal complaints by patients, relatives, or carers in the course of day-to-day duties, in line with local policy and procedures and escalate concerns you are unable to resolve to an appropriate person.
18. Ensure all documentation and record keeping is accurate, legible, and completed in a timely manner, in accordance with NMC and local policy.
19. Maintain effective links with community, intermediate services, and other nurse practitioners, utilising their skills and knowledge.
20. Actively participate in professional forums including Practice meetings, multi-professional meetings.
21. Have the courage to raise concerns regarding quality and patient safety and act upon/escalate these, following relevant national and local policy.

* Full UK Driving License and Access to own car including business insurance
* Non-Medical Prescribing Qualification
