This is an ideal opportunity for a Registered Nurse who is seeking to broaden and utilise their range of skills and knowledge to nurse people in their own environment as part of a multi-disciplinary team. The Community Staff Nurse is responsible for the delivery of prescribed nursing care to patients on a day-to-day basis. This includes the holistic assessment of patient care needs and the development, implementation, and evaluation of evidence-based care. Duties include palliative care support and assessment, care at the end of life, supporting and assessing individuals and carers' needs with long-term health conditions, promoting self-management skills, and wound care. The post holder will be autonomous in their clinical practice while acknowledging their limitations and actively seeking advice as necessary. The Newtown District Nursing Team is an integrated part of the virtual ward that aims to prevent unnecessary admissions to hospital and support early discharge from hospital.
