The post holder will be an experienced Nurse Practitioner who, acting within their professional boundaries, will provide assessment, diagnosis, and treatments to acute episodes of minor health conditions as well as minor injuries. The ANP will be expected to see patients autonomously providing face to face, Telephone consultations within the practice and home visits within the patients residence and demonstrate the use of critical thinking in the clinical decision-making process. You will work collaboratively with the general practice team and meet the needs of the patients, supporting the delivery of policy and procedures and providing Nurse leadership as well as promoting self-care to our patients. Main Duties: Assess, diagnose, plan & implement care for patients. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly. Diagnose and manage both acute and chronic conditions, intergrating both drug and non-drug based treatment methods into a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs in accordance to National and Local guidelines. Work with patients in order to support compliance with and adhere to prescribed treatments. Provide information and advice on prescribed and over the counter medication regarding regimens, side-effects and interactions. Prioritise health problems and intervene appropriately to assist the patient in all situations. Support patients to adopt health promotion strategies and apply principles of self care.
