As Clinical Pharmacist and Independent Prescriber, you will work as part of the Clinical Pharmacy Team within Suffolk Primary Care, to improve the quality and safety of care offered to the patients of Combs Ford Surgery. You will have regular contact with patients and will help seek to reduce the workload of GP prescribers in the practice, whilst also supporting the management of patients with long-term conditions.
You will work as part of a multi-disciplinary team to manage medicines on transfer of care and systems for safer prescribing. You will perform face-to-face medication reviews of patients with polypharmacy; especially for those with frailty and/or with multiple co-morbidities. You will undertake face-to-face reviews of patients with specific long term conditions that fall within your competency. You will provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship. You will contribute to practice financial stability through optimisation of medicines related targets in QOF.

As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process.
9. To attend local, regional and national meetings of relevance as agreed with the post holder's line manager.

11. Attend monthly QI audit meetings and carry out/co-ordinate the monthly QI audits themselves.
12. Being responsible for overseeing the day-to-day operations of the Dispensary and managing its staff.
13. Checking the drug tariff on a monthly basis for any price or category changes, as well as identifying new or discontinued drugs. Contacting drug company representatives when needed to address changes.
14. Working alongside the designated partner to update and maintain the computerised Drug Directory. This includes adding new medications and monitoring patient prescription refills.
15. Collaborating with pharmaceutical advisors from commissioning organisations.
16. Organising prescriptions, reviewing endorsements, completing necessary forms, and submitting prescriptions monthly to the Prescription Pricing Authority (PPA). Also, compiling a list of vaccines and handling prescriptions sent back by the PPA.
17. Completing assigned tasks, preparing reports (e.g., annual DSQS), providing support for projects, and participating in development or change initiatives as they arise.
18. Processing of letters assigned to the dispensary team.
19. To develop and drive forward profitability pathways.
20. Ensure full compliance with dispensing law and regulations. Maintain good probity in all dispensing activities.
Line Management responsibilities:
1. Address any issues raised by fellow members of the Dispensing team.
2. Organise schedules for dispensary staff and ensure there is adequate coverage at all times.
3. Plan, lead, and document monthly meetings for the Dispensing Team.
4. Develop, keep current, and ensure awareness of all Dispensary Standard Operating Procedures among team members.
5. Promote and facilitate the personal growth of Dispensing team members. Maintain a professional demeanour and uphold high service standards.
6. Keep the Practice Operations Manager informed about emerging matters, issues, or potential concerns.
7. Conduct staff appraisals for the Dispensing team, guided by the Practice Operations Manager.
8. Assist the Practice Operations Manager in addressing disciplinary matters involving dispensary staff.
9. Be ready to take on shifts and assist with filling dossett boxes as needed.

* Working with Older People with Frailty and vulnerable patients
* Previous Management of medicines at discharge from hospital including patient cohorts who attend with medicines related issues. E.g. Diabetic patients presenting with hypoglycaemia.
* Experience giving Clinical advice and expertise on medicines to practice staff and patients
