Location: You will work across both of our sites in Hook and Hartley Wintney.

Purpose:
* The post holder will act as a clinical pharmacist to our patients, working under guidance from the nurse Manager and GP team and will be involved with the care of acute and routine patients.
* Utilise their clinical knowledge in the management and optimisation of medicines while also continuing to provide core clinical pharmacist duties.
* The post holder will provide clinical leadership on medicines optimisation and quality improvement.
* The post holder will ensure the execution and delivery of safe, efficient and effective pharmacy services to improve patient health and care, promoting high quality, cost-effective and safe prescribing in their environment and assist in the development of future services.
* The post holder will develop and build relationships with other staff.
* To provide training as required to other clinical pharmacists and also to other disciplines.
Responsibilities:
Responsibility for Patient/Client Care:
* Consult with patients with single or multiple medical conditions where medicines optimisation is required or medication review is needed.
* Support the duty clinical team with acutely unwell patients with prescribing needs and health and well-being education.
* Manage own case load and run long-term condition clinics where responsible for prescribing as an independent prescriber for conditions where medicines have a large component.
* Review the ongoing need for each medicine, monitoring needs, and support patients with their medicines taking.
* Identify patients needing high-risk drug monitoring and ensure regular reviews occur.
* Undertake structured clinical medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes.
* Provide telephone consultations for patients with questions, queries, and concerns about their medicines.
* Implement practice searches to identify cohorts of patients most likely to be at risk of unplanned admissions and readmissions from medicines.
* Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients.
* Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care.
* Produce and implement a practice repeat prescribing policy.
* Provide pharmacist and prescribing support to staff including Dr’s and Advanced Nurse Practitioners.
* Provide clinical support and professional supervision to pharmacy technician workforce.
* Liaise and work with the North Hampshire ICB Medicines Management Team.
* Conduct medicine reconciliations from discharge letters.
* Have a good working knowledge of targets such as QOF, IIF and implement measures to maximise achievement.
Communication & Relationship Skills:
* Demonstrate effective clinical leadership and promote proactive and positive behaviours.
* Coordinate workload between colleagues to ensure safe coverage and encourage learning.
* Participate in maintaining systems for risk assessment.
* Contribute to investigation of incidents and complaints as required.

* Monitor standards of care through quality markers and benchmarking.
* Work proactively in managing change to improve practice and health outcomes.
* Identify and provide leadership on areas of prescribing and medicines optimisation.
* Present results and provide leadership on suggested changes.
Analytical, Judgemental Skills, Planning & Organisational Skills:
* Analyse, interpret and present medicines data to highlight issues and risks.
* Provide leadership to ensure compliance with CQC standards.
* Undertake risk assessment and management.
* Support public health campaigns and provide specialist knowledge.
