Job summary An exciting opportunity has arisen for an enthusiastic Band 3 Community Health Care Assistant to come and join us in our Community District Nurse teams in Solihull. The band 3 Health Care Assistants role is to assist, without direct supervision, in the delivery of high standards of nursing care to patients in the community by following prescribed programmes of care. The role involves working autonomously in the community so you must be a car driver/owner. This post requires a motivational and enthusiastic individual who is passionate about patient care. You must be flexible, approachable, compassionate, motivated, and hardworking and be able to use your own initiative. The post holder will be expected to work days and weekends including bank holidays, 37.5 hours per week. The service operates between 07.00 and 23.00 hours, 7 days a week and you will be required to work shifts between these times. Part time/Flexible working will be considered. To support the qualified members of the nursing team in providing high standards of holistic and personalised patient care in the community. 2. To work collaboratively with GPs and other professionals and agencies to ensure the best possible care is provided to patients in the community, avoiding unnecessary admissions and prolonged hospital admissions. 3. To perform simple delegated nursing procedures following completion of appropriate training and competency assessment. 4. To report to caseload holder/ Staff Nurse any changes in patient condition or relevant information pertinent to patient care. 5. To fit and demonstrate the safe use of equipment, educate and advise patients/ carers in the correct use of equipment as directed by the qualified nurse. 6. To administer prescribed medication in line with Trust Policy. 7. To plan and prioritise own visits to delegated patients. 8. To communicate effectively with patients and their family/ carers, colleagues and other professionals and agencies. 9. To ensure that risk factors to clients, carers and Trust staff within their working environments are recognised and reported. 10. To work in accordance with the Trust's policies and procedures. 11. To ensure that accidents, complaints and incidents are reported in line with the Trust's policy. 12. To participate in the annual appraisal process for self. 13. Maintain accurate, contemporaneous written and computerised records for all patients having due regard to their legal status.
